The MVD requires the submission of this waiver request to attend traffic survival school via Zoom. Please submit it promptly to ensure quick approval. (This waiver request does not waive the fee.)
There are two ways to submit a waiver. You can submit via email or by phone, and the instructions for both methods are as follows:

Option One: Via Email
On the registration form, please respond to the question “Do you have a waiver?” by selecting the option “I do not have a waiver submitting via email.” The Motor Vehicle Division (MVD) generally requires 3 to 5 business days to process this request, so be sure to select a date accordingly. You will receive an email within 24 hours with instructions on obtaining a waiver. Completing the waiver form will only take a few minutes of your time.
Option Two: By Phone
On the registration form, please respond to the question “Do you have a waiver?” by selecting the option “I do not have a waiver submitting by phone.”
To obtain your waiver by phone, call the MVD at (602) 255-0072. Ask to speak with a tier-two representative to submit a virtual waiver for traffic survival school.
If you are asked for a reason, you can explain your situation, such as not having a license and lack of transportation to attend the in-person class, or facing work-related, family, financial, or health issues, etc.
Once your waiver is submitted, you should receive approval within 24 to 48 hours. The MVD will upload your approval letter to your AZMVDNOW.GOV account.
